Sports Sign-up Checklist
Registration – Athletes must obtain the following in order to receive a yellow eligibility card* :
Current physical Low cost physical Athletic Participation Form Athletic Agreement Form Athletic Fees/ASB cards * NO CARD, NO PRACTICE, NO EXCEPTIONS

Current Physical - Have office staff check your record	Top
Current for two years Must remain current for entire sport season If expired/expiring – YOU MUST GET A NEW ONE Forms can be obtained in the guidance office According to Oregon State Law, your physical must be completed on this form: Physical Form
Low Cost Physical- August 12	Top
Low Cost physicals are available. You must bring the physical form with you as they will not be provided. Where: McKay High School When: August 12 from 4:30 pm to 7:00 pm Cost: $35.00 per athlete

Athletic Participation Form - Obtain at Guidance Office	Top
Both parent and athlete must sign Fill out entire form—especially yellow highlighted areas Include health insurance info – (insurance can be purchased, ask office)

Athletic Agreement Form - Obtain at Guidance Office	Top
Both parent and athlete must sign

Fees - Please pay bookkeeper and hold onto receipts	Top
$100.00 per athlete per sport (see handbook for more details) ** $ 40.00 for ASB card

** If you are on free/reduced lunch program, fees are as follows:
$40.00 for reduced lunch $20.00 for free lunch Forms can be picked up at Guidance Office.

If a student is NOT selected for a team, which uses the selection/cut process, the participant fee is refunded to the student.
